Non-Dual Enrollment Students may request to either Late Drop or Late Withdraw courses by using this petition form. (Specific criteria apply.)
NOTE: If you are a dual enrolled student, use the "Late Drop or Withdraw - DUAL ENROLLMENT STUDENTS ONLY" form. Speak with the Director of Accelerated Pathways prior to submitting any petition. All dual enrollment petitions require multiple approvals including from the school and district administration.
This form is to be completed by faculty to initiate a late attendance verification for a student.
If you are getting your personal computer worked on by an FSW Helpdesk technician, you will need to complete this request.
