Our Application Fee Waiver program is designed to assist those who may face financial barriers in paying the application fee. Follow the simple steps below to request a waiver for your FSW admission application fee.

Advisors: Begin this form to assist a student with planning their transient (“guest”) enrollment at a private or out-of-state institution – an institution that is not a part of FloridaShines.
Non-Dual Enrollment Students may request to either Late Drop or Late Withdraw courses by using this petition form. (Specific criteria apply.)
NOTE: If you are a dual enrolled student, use the "Late Drop or Withdraw - DUAL ENROLLMENT STUDENTS ONLY" form. Speak with the Director of Accelerated Pathways prior to submitting any petition. All dual enrollment petitions require multiple approvals including from the school and district administration.
This request form allows FSW staff to submit a request for the creation or modification of financial detail codes, such as those for charges or payments related to fees, and other institutional costs.
Students may request a waiver of the Multiple Attempt Surcharge by using this petition form. (Specific criteria apply.)
Custodial service requests include sanitation issues/requests for bathrooms, interior garbage/recycling, carpet cleaning and vacuuming, as well as windows.
Use this service form to request access to the college's reporting tool, Argos.
Services provided by Building Maintenance may include items such as plumbing, installations, painting, repairs, outdoor trash clean-up, graffiti removal, and parking lot/sidewalk maintenance.
Students may request to exclude the GPA associated with previous poor grades by using this petition form. (Specific criteria apply.)
