After you submit this form, you’ll receive an email letting you know if you are eligible and if equipment is available. Requests are processed on a first‑come, first‑served basis. Please bring your FSW ID card when picking up equipment on campus.

By checking out this equipment, you acknowledge responsibility for any repair or replacement costs as described in the Student Loaner Technology Acceptable Use Policy (AUP).
