Former students, who no longer have access to their FSW BUCS email account, must present the form in person at an FSW campus location or attend a scheduled ZOOM appointment with a member of the Registration team to verify identity.
Current students may submit the form through their FSW BUCS account by using the Current Students option, or in person, to update their data through the Office of the Registrar. Please be aware you may need to further verify your identity and present your verification documents in person at an FSW campus location or attend a scheduled ZOOM appointment with a member of the Registration team before changes can be completed.
