- Other Reason: Specify your reason and provide appropriate documentation.
What Happens Next?
Once you submit this form and attach the required documents, our Office of Admissions will review your request. If approved, you will receive a unique promo code via the email address provided. This code should be entered into your online application to waive the fee before the application deadline.
