Locating the Ground Course Test Request Form

Faculty may submit requests for ground course make-up exams and testing with disability-related accommodations through the official Ground Course Test Request form.

To submit a new Ground Course Test Request form, navigate to the link above and select "Request Service."

You may be prompted to sign in. If so, enter your FSW credentials to continue.

Completing the Form

Complete the form using the drop-down menus to enter the pertinent information. You will be asked to provide the Course, Test Name, Campus location, and your Department.

Next, provide the test Open and Close Dates, as well as the allocation of time you would like to provide the student to complete the exam. 

If the student will need testing accommodations, you can continue reading this guide for information about testing with accommodations.

Finally, fill out the student's name and email and select "Submit" to complete the form.

Requesting Disability-Related Accommodations

If the student will require disability-related accommodations, you can simply check the boxes next to the required accommodations.

Please be sure to review the "Rules for Testing" section carefully for information about the amount of advance notice Testing Services will need to schedule the exam, the latest time an exam can be scheduled that day, and which forms of ID are accepted.
