External Organization Logo Request

This form is to be used by non-FSW-affiliated organizations to request logos for external webpages or publications.

Acknowledgment of Use for Florida SouthWestern State College (FSW) Logo

By submitting this request, I/we (the requestor) acknowledge and agree to the following terms for the use of Florida SouthWestern State College’s (FSW) logo for non-commercial purposes:

  1. Permitted Use:
    The logo is authorized for use solely for the specific project/event outlined in this request (e.g., sponsorship flyers, presentations). Any other use requires separate approval from FSW.
  2. Non-commercial Purposes:
    The logo will be used exclusively for non-commercial purposes. I/we will not sell, license, or otherwise commercialize the materials featuring the FSW logo.
  3. Brand Guidelines:
    The FSW logo will be used in accordance with FSW’s official Brand Guidelines. The logo must not be altered or modified in any way, including but not limited to changes in color, proportions, or design.
  4. Prohibited Use:
    The logo may not be used in any way that suggests endorsement, sponsorship, or affiliation with FSW without prior written consent. Additionally, it will not be used in any context that may reflect negatively on the college, including offensive, inappropriate, or controversial materials.
  5. Acknowledgment of Ownership:
    The FSW logo is the intellectual property of Florida SouthWestern State College. I/we acknowledge that FSW retains all ownership and rights to the logo and may revoke permission to use the logo at any time.
  6. Liability:
    I/we agree to indemnify and hold harmless Florida SouthWestern State College from any and all claims, damages, or liabilities arising out of or in connection with the improper use of the logo.
The due date indicates the date that the work on the ticket is to be completed. If the ticket has estimated hours associated, the due date is the end of the resource allocation range.
By acknowledging the above, I/we confirm that I/we have read, understood, and agree to comply with the terms for the use of FSW’s logo.
