- Have you ever been charged for, even if adjudication was withheld, behavior involving harm to others (such as violence, stalking, domestic violence, assault, or robbery) or a sexual offense (such as lewd or lascivious acts, rape, sexual battery, or unlawful sex with minors)?
You will need to fill out the Criminal/Disciplinary Disclosure Form and provide appropriate supporting documentation. Failure to provide a complete statement or all of the necessary documentation will delay an admissions decision.
