Criminal/Disciplinary Disclosure Form

You have received the link to this form because our records indicate you answered "Yes" to one or both of the following questions on your application for admission:

  1. Have you ever been charged with/or subject to disciplinary action for scholastic (such as plagiarism or cheating) or any type of behavioral misconduct at any education institution? (For poor academic standing, you do not need to disclose academic dismissal, suspension, or probation.)
  2. Have you ever been charged for, even if adjudication was withheld, behavior involving harm to others (such as violence, stalking, domestic violence, assault, or robbery) or a sexual offense (such as lewd or lascivious acts, rape, sexual battery, or unlawful sex with minors)?

 

You will need to fill out the Criminal/Disciplinary Disclosure Form and provide appropriate supporting documentation. Failure to provide a complete statement or all of the necessary documentation will delay an admissions decision. If this was a mistake on your application, please answer "No" to both questions and complete the Student Acknowledgement section at the bottom.
