This form is to be completed by faculty to initiate a late attendance verification for a student. All of the following qualifiers must be true before submitting.
- The student missed the deadline for the Attendance Verification assignment.
- The student is attending class.
- The date the Office of the Registrar will receive this completed form is prior to the 60% date of the relevant semester, mini-terms included.
If the date is beyond the 60% date of the relevant semester as listed on the Official Academic Calendar, this form cannot be submitted, and attendance cannot be verified. For advisement, contact your Dean.
