Points To Note:


  • The form times out if not submitted within 30 minutes.
  • In order to allow an appropriate time frame for students to schedule their appointments, we request that faculty submit their test requests as soon as possible, but a minimum of two (2) business days prior to the date in which the exam is to be administered (i.e., “open” test date).
    • When selecting open/close dates, please be aware that proctoring is not available on Saturdays or Sundays.
  • Accurate completion of form information ensures students receive the best possible testing environment and reduces the need for a follow-up.
    • This is true, especially in the case of valid student emails. Upon form submission, auto-generated emails are sent to Professors/Students/Testing Staff/Adaptive Services Staff. Faculty can refer students to view their emails for specific testing information, relieving faculty of the need to follow up on these types of details—link to make an appointment, testing locations/hours, exam open/close dates, test aids allowed, time limit of the exam, etc.
  • For those faculty who have students requiring accommodations, it is essential to store the Letter of Accommodations (LOA) in a secure access file, as you will be required to select the appropriate drop-down options per LOA for each submission.
  • Don't forget to upload your exam and any test aids. You may upload up to 5 Word and PDF documents to this form.

  • Don't forget to click on the "Submit" button at the bottom of the form.
  • You may submit another exam request on the "Success" notification page.
