3. Complete the Verify Identity Form

You will need to complete the password reset form to confirm your identity and create a new password. All fields are required, except for one: you may provide either your FSW ID number OR the last four digits of your social security number. You do not need to enter both.

4. Create a New Password

Enter your new password and click "Set Password".

Your password must meet the following requirements:
1. 14 to 20 characters in length. It must contain at least one lowercase letter, one uppercase letter, and one number.
2. Does not contain personal information. For example, first name, last name, or date of birth.
3. Previously used passwords will be rejected.

5. Username, FSW ID Number, and Sign In to MyFSW

Your username and FSW ID number will appear on the "Success" page. Please note down this information as you will need your username to log into MyFSW.

Note – Do not share your password or MFA codes with anyone. No third party is allowed to have access to your account.

Click "Log into MyFSW". Next, enter your username and new password and click "Sign In".
